Physical/Occupational Therapy
Educational Programs & Services

Physical and Occupational therapy are related services that are available to students with Individualized Education Programs (IEPs) or Service Agreements who need PT and/or OT to benefit from education.

Services available through the PT/OT Program:

  • Evaluation of students to determine the need for PT/OT services.
  • Parent and teacher training and support.
  • Individual therapy in schools by licensed therapists.
  • Consultation with school personnel about disabilities, development, adaptions and assistive devices.
  • Assistance with the development of IEP goals and objectives.
  • Completion of ACCESS (medical assistance) forms.

To refer a student for a PT and/or OT evaluation:

  • Complete a PT / OT referral form.
  • Attach a "permission to evaluate" form signed by the parent.
  • If the parent has submitted a physician's prescription, enclose it with the referral.

Please note that if the outcome of a PT/OT Evaluation is a recommendation for services, a physician's prescription must be obtained (and renewed annually) before services can begin.
